| - First time office visit is $120 for one hour with a physician. |
| - Return office visit is $85 for 45-60 minutes with a physician. |
| - The cost for brief visits of less than 45 minutes duration is determined by the physician according to the length of time taken and the complexity of the evaluation and treatment involved. |
| - Additional charges may include laboratory fees & medicinary products. |

| Phone Consultations |
| When an office visit is not necessary or possible, we often consult with our established patients over the phone. The fee for these consultations is determined by the physician based on time and complexity in each case. |

| Cancellation Policy |
| Notice of cancellation of scheduled office visit is required 24 hours in advance to avoid a $25 charge. |

| Payment is required at time of visit. |
